How to apply for an Individual Number CARD マイナンバーカードの申請方法
Smartphone
- Scan the QR code printed on the application form with a smartphone camera. カメラで交付申請書のQRコードを読み取る。
- Go to the application website and register your e-mail address. 申請用webサイトにアクセスし、メールアドレスを登録。
- Take a photo of your face and register it, enter details as required. スマートフォンのカメラで撮影した顔写真を登録、必要事項を入力。
Personal computer
- Go to the application website and register your e-mail address. 申請用WEBサイトにアクセスしてメールアドレスを登録。
- Go to the application website through the link that is sent to your registered e-mail address and register your face photo taken by a digital camera. 登録されたメールアドレス宛に通知される申請者専用 WEBサイトにアクセスし、デジタルカメラで撮影した顔写真を登録。
- Enter the remaining application information as required. その他申請に必要な情報を入力。
ID photo booth
- Select “Apply for the Individual Number Card” from the touch panel and insert money for the photograph and place the QR code for the barcode scanner. タッチパネルから「個人番号カード申請」を選択し、撮影用のお金 を入れて、交付申請書のQRコードをバーコードリーダーにかざす。
- Follow the on-screen guidance and enter necessary items, take an ID photograph and send. 画面の案内にしたがい、必要事項を入力し、顔写真を撮影して送信。
- Sign or write your name along with a personal seal and paste your photo on the application form. 交付申請書に署名または記名・押印し、顔写真を貼り付ける。
- Make sure you have filled in all the required fields before you post it. 交付申請書の内容に間違いがないか確認し、送付用封筒に入れて、郵便ポストに投函。

If you have extended your period of stay… An update to the information printed on your Individual Number CARD is required. 在留期間の延長を行った場合…マイナンバーカードの券面記載事項の変更が必要です。
your Individual Number CARD will become invalid if you fail to extend it at your local municipal office before its expiration date. マイナンバーカードの有効期限が経過するまでに市区町村の窓口で延長の手続を行わないと、カードは失効してしまいます。
